ED Medications: Cialis, Levitra, Viagra

By blocking PDE V temporarily, these medications empower the chemical, Nitric Oxide (NO) that occurs naturally with arousal to initiate, produce and maintain an erection. Patients need sexual stimulation and foreplay to release NO initiate an erection in order for these medications to be effective. This class of medication is a safe and effective oral treatment for men with erectile dysfunction of physical, psychological or mixed cause. However, it is important that patients should be aware of the low probability (less than 50%) that intercourse will be possible after the first dose – particularly in severe or advanced cases and especially when using Viagra or Levitra.

Cialis, Levitra (Vardenafil) and Viagra™ (Sildenafil) work by selectively inhibiting an enzyme in the penis called Phosphodiesterase V (PDE V), a chemical that takes away the erection. The majority of men who stop Viagra because of apparent lack of effect will in fact respond and achieve intercourse if they continue to try again, progress from 50 mg to 100 mg, or take the pill without food on an empty stomach (3 hours after eating). Taking Viagra or Levitra on a full stomach will not only reduce their peak concentration in the body by 30 to 50% but in addition will delay their peak concentration by one hour.

In other words one may have to wait after intake before initiating sexual activity. Cialis peak concentration and time before reaching maximum absorption is not affected by a full stomach. Because of this it remains my drug of choice for the treatment of ED and I always prescribe it first. In addition many of my patients experience fewer side effects with Cialis then with the other two.

• Viagra (sildenafil): Most common ED pill, effective within 30-60 mins, lasts 4-6 hours, requires prescription.
• Cialis (tadalafil): Long-lasting, up to 36 hours, can be taken daily, suitable for frequent use.
• Levitra (vardenafil): Similar to Viagra, effective within 30-60 mins, lasts about 4-5 hours.
• Stendra (avanafil): Fast-acting, effective within 15 mins, fewer side effects, prescription required.
• Staxyn (vardenafil ODT): Dissolves on tongue, quick onset, similar to Levitra, no water needed.
• Vidalista (tadalafil): Suitable for daily use, longer duration, used for BPH as well.
• Kamagra: Generic sildenafil, similar effects to Viagra, caution advised due to unregulated sources.
• Spedra (avanafil): Rapid onset, good for spontaneous sex, fewer side effects.
• Cenforce: Popular generic sildenafil, similar effects, often more affordable.
• Tadalafil Daily: For consistent erectile function, taken once daily in lower doses.
• Sildenafil Chewable: Faster absorption, convenient for those who have trouble swallowing pills.
• Herbal Supplements (e.g., Horny Goat Weed): Natural alternative, limited scientific evidence, consult doctor first.

Finally Cialis has a long period of activity and patients are successful at intercourse up to 36 hours after taking this pill. This represents a distinct advantage from the patient’s perspective as it diminishes the need for planning. Sex can be more spontaneous and natural.

Typically, medication is the first treatment recommended to those with erectile dysfunction.

What erectile dysfunction pills are available?

Many men experience relief of symptoms after using medication. Our doctors use the results of diagnostic tests to determine which medication is best suited for you. The first treatment usually offered to men with erectile dysfunction is a prescription medication, such as sildenafil (Viagra®), tadalafil (Cialis®), or vardenafil (Levitra®). Generally taken 30 to 60 minutes before sexual activity, these medications work by improving blood flow to the penis during arousal. Erectile dysfunction medications like Viagra® should not be used by men who take nitrate medications to treat the symptoms of heart disease.

Which Medication Will Work Best for My ED?

Erectile dysfunction medications can cause a drop in blood pressure. When combined with nitrates, which dilate or open up the arteries to the heart to increase blood flow, there is the potential for a severe drop in blood pressure. Some 35 percent of men do not respond to oral medications for erectile dysfunction. A smaller percentage of men may experience uncomfortable side effects, such as flushing or feeling suddenly warm, changes in vision, upset stomach, or heartburn. If, after several months, you have not seen an improvement in these side effects, your doctor may offer other medical treatment options. Having sex long after taking the pill makes men feel more normal! For these reasons I will first try Cialis on all my Patients and reserve Viagra and Levitra for those men who do not tolerate Cialis.

• Cialis for Daily Use: 2.5mg or 5mg taken daily, maintains steady medication levels in the bloodstream.
• Levitra 20mg: Higher strength for patients needing a more potent vardenafil effect.
• Kamagra (Unofficial Sildenafil): Not FDA-approved, often sold online, carries significant safety risks.
• Heart Health Assessment: Essential first step, ED can be an early sign of cardiovascular disease.
• Drug Interaction Review: Crucial before starting any ED pill, especially with nitrates or alpha-blockers.

The steps illustrated in Reference #4 below will help optimize the chances of success of Levitra and Viagra4. Reliable safety data gathered in the 10 years since these PDE % inhibitors were launched confirm that side effects such as Priapism (prolonged erection), red eyes, painful eyes, syncope(fainting), tachycardia (rapid heart rate), and nausea do occur, but are rare. Back pain and muscle aches occur rarely with Cialis. HIV medications (protease inhibitors) have been shown to raise their blood levels. Certain patient groups are more susceptible to a decrease of their blood pressure, including those with aortic stenosis, hypertrophic obstructive cardiomyopathy, and multiple system atrophy.

Levitra (Vardenafil)

Intraurethral medication is often used as an alternative in men who don’t respond well to oral medications for erectile dysfunction. This is a suppository inserted through the opening in the penis. Using a narrow, prefilled plastic applicator, the man inserts the medicine directly into the urethra 5 to 10 minutes prior to sexual intercourse. The medication causes the muscles of the penis to relax and the arteries to widen, enabling adequate blood flow for an erection. Some men experience side effects from intraurethral medications, such as dizziness, a burning sensation in the urethra, or an aching feeling in the penis, legs, and pelvis.

What are the side effects of ED pills?

These generally go away when the medication leaves the body. Because some men experience dizziness and, rarely, fainting, within an hour of using these medications, your doctor may recommend that you refrain from driving or operating heavy machinery. Penile self-injections were the first medication approved by the U.S. Food and Drug Administration for the treatment of erectile dysfunction. These are prescribed for men who don’t respond well to oral medications.

Vacuum Devices

An NYU Langone urologist performs the first injection in the doctor’s office, in order to determine the dose of medication needed. Your doctor then instructs you on how to safely inject the medication, which is supplied in vials with syringes, for use at home. Typically, a man injects the medication into the shaft of the penis 10 to 20 minutes before sexual activity to achieve an erection. Side effects can include an aching sensation in the penis, which generally goes away in a few hours. In some men, medical treatments for erectile dysfunction may occasionally cause a prolonged erection. They are in a class called PDE 5 inhibitors. They work to restore a normal system of blood flow in the penis so that an erection can occur with sexual stimulation. Only ED medicine clinically proven to work up to 36 hours. Works as fast as 30 minutes in some men.
